During my examination, he showed clear signs of ACL instability through tests like the Lachman and anterior drawer tests. His X-rays were normal, and his MRI revealed a mid-substance tear of the ACL—essentially a tear in the middle of the ligament.

Here’s where it gets critical: the ideal candidate for ACL repair is someone with a proximal tear (a tear near where the ligament attaches to the femur) and robust, healthy tissue. Unfortunately, this patient’s tear wasn’t a textbook case for repair.

Weighing Risks vs. Benefits

I explained to him that, while ACL repair is less invasive, it does not have the decades of proven success that ACL reconstruction offers, especially for high-level athletes.

  • ACL Reconstruction: Well-established, high likelihood of returning to professional-level athletics, excellent long-term outcomes.
  • ACL Repair: Newer technique, promising for some patients, particularly recreational athletes, but long-term data for elite athletes is limited.

Because of the patient’s high-level athletic demands and the nature of his tear, I recommended reconstruction. He ultimately agreed, and I’m happy to report that he is recovering well.
